At the time of your visit, our receptionist will ask you to complete a Patient Information form. You will be asked to sign an authorization form for release of medical information and for your insurance company. All information you provide us is strictly confidential. If you are more than 15 minutes late for your appointment we will need to schedule you another day.

Please bring with you to your visit:

• All Medications you are currently taking.
• Insurance cards.
• Proof of income if you will be applying for the sliding fee (paycheck stubs or tax returns).

Family Health Center of Southern Oklahoma provides high-quality, affordable health care. Our doctors and staff are committed to keeping you and your family healthy at rates you can afford. To continue with our current level of service, it will be necessary to collect the fee from all of our patients when services are received.

Uninsured patients can pay on a sliding fee scale according to income. Proof of household income is required.
