Become A Patient

The Family Health Center of Southern Oklahoma allows self-check-in for patients up to 48 hours before their appointment; simply request advanced check-in when scheduling your appointment.

FHCSO also strongly encourages our patients to join the Patient Portal. This portal enables secure communication with the care team, accepts submissions for medication refill requests, and allows access to lab results. To register, a valid email address is required. For assistance, please reach out to a staff member.

Sliding Fee Application Information

We offer a sliding fee scale for some services, which adjusts costs based on income and household size to help make care more accessible. Please note that eligibility for the sliding scale is limited and not guaranteed for all clients or services. If you’re interested, we’ll review your information during the intake process to determine if you qualify. Standard rates may still apply depending on your situation.

Feel free to contact us with any questions or to learn more about eligibility.

In compliance with the No Surprises Act, FHCSO offers Good Faith Estimates to Uninsured patients for appointments scheduled 3 or more days in advance. Good Faith Estimates will be sent from Family Health Center of Southern Oklahoma (FHCSO) within 24 hours of scheduling an appointment or specifically requested.

Good Faith Estimates will be determined based on the described chief complaint at the time of scheduling an appointment. The health center will, to the best of its ability, provide detailed codes and related cost of service. Good Faith Estimates will be valid for no longer than one (1) year from the date of publication. If actual costs exceed $400 of the estimate, patients may request reconsideration of actual charges or request independent arbitration.

FHCSO kindly requests patients to enroll in the Patient Portal for the most efficient delivery of Good Faith Estimates. This portal also includes access to additional Health Information including Lab Results, Current Medications and the ability to Securely communicate with your provider.
