Patient Information

At the time of your visit, our receptionist will ask you to complete a Patient Information form. You will be asked to sign an authorization form for release of medical information and for your insurance company. All information you provide us is strictly confidential. If you are more than 5 minutes late for your appointment we will need to schedule you another day.

Please bring with you to your visit:

  • All Medications you are currently taking.
  • Insurance cards.
  • Proof of income if you will be applying for the sliding fee (paycheck stubs or tax returns).

Family Health Center of Southern Oklahoma provides high-quality, affordable health care. Our doctors and staff are committed to keeping you and your family healthy at rates you can afford. To continue with our current level of service, it will be necessary to collect the fee from all of our patients when services are received.

Ashley Rogers, MSW, LCSW

Ashley Rogers serves as the Behavioral Health Director at FHCSO in Tishomingo, where she is dedicated to providing compassionate, patient-centered care. A Tishomingo native, Ashley is passionate about creating a safe, supportive space for individuals to share their stories, feel heard, and find their voice.

Ashley earned her Bachelor of Social Work from East Central University in 2011 and completed her Master of Social Work at the University of Oklahoma in 2013. She became licensed through the Oklahoma State Board of Licensed Social Workers in 2017. Throughout her career, she has worked with individuals across the lifespan, with extensive experience supporting the geriatric population.

Outside the clinic, Ashley enjoys life with her husband, their three children, and their two golden retrievers, Milly and Remy, FHCSO’s beloved therapy dogs. She can often be found cheering on her kids at their sporting events, spending time outdoors with her family, or enjoying the Oklahoma countryside through hunting and outdoor activities.

Devon Gilbert

Devon Gilbert was born and raised in Tishomingo, where she graduated high school in 2009. She recently moved back to Tishomingo after living in Norman, Oklahoma. Devon graduated from East Central University in 2014 with a Bachelor of Social Work degree, and a minor in psychology. Devon continued her education at the University of Oklahoma where she obtained her Master of Social Work in 2015. She completed her supervision requirements in 2020, earning her LCSW, and has since been practicing as a Licensed Clinical Social Worker. Devon enjoys spending time with her family, trying new restaurants, and traveling to the mountains.

Melissa Blackburn, LPC, PMH-C

Melissa Blackburn, LPC, PMH-C, is a Licensed Professional Counselor with over a decade of comprehensive experience serving children, teens, and adults. Drawing from her deep Oklahoma roots, Melissa strives to create a safe and supportive environment where individuals and families can heal and thrive. She is focused on teaching social-emotional regulation skills, emphasizing that strong emotional health is vital for overall well-being. Melissa holds a key certification in Perinatal Mental Health (PMH-C), guiding mothers and fathers through recovery during a vulnerable life transition. Her extensive expertise also covers trauma recovery for children (TFCBT certified) and adults (CPT), general wellness (ACT, CBT), and strengthening relationships (Gottman Method-techniques). Melissa’s mission is to help others grow their perspectives, improve communication, and develop healthy habits that lead to more meaningful and fulfilling lives. She is also an approved LPC Supervisor, actively mentoring new professionals in the field. When she’s not in the office, you can often find her outdoors, chasing her kids around and staying active with her family.

Joy Gibson

Joy Gibson is originally from Tennessee, but has been in Coalgate for almost 18 years. She received her bachelor’s degree from a small college in Tennessee, Bethel College, and about 10 years later received her Master’s from East Central University. Joys passion is helping others be the best they can be, getting through rough times side by side until they are able to fly on their own. She has two teenage boys, and has recently gained a husband and two more boys for a FULL house. Joy loves watching her boys play ball, relaxing in her hammock, spending time with friends, and simply just being at home with her family.

Tisha Beshirs

Tisha Beshirs is a Licensed Professional Counselor serving the Kingston community through FHCSO’s Behavioral Health Team. She holds both undergraduate and graduate degrees from Northwestern Oklahoma State University, where she earned a Master’s in Counseling Psychology. Following the completion of her graduate program, she fulfilled the requirements to become a Licensed Professional Counselor..

Tisha brings extensive experience to her role, having previously worked with Oklahoma Children’s Services and the Department of Human Services before transitioning into clinical counseling. Prior to joining FHCSO, she provided therapy services at two other community-based counseling agencies in the region.

Originally from Canton and Oakwood, Oklahoma, Tisha is deeply committed to supporting individuals and families through life’s challenges. She is passionate about helping clients navigate difficult times and empowering them to rediscover hope and resilience. Outside of work, Tisha and her husband, Shannon—Chief of Police in Kingston—stay active in the community. They enjoy spending time with their large, blended family, attending school events, and participating in activities with the Blue Knights Motorcycle Club, Ardmore Chapter. In their free time, the Beshirs family enjoys cooking out and fishing together.
